The regular season ended with a thrilling night of action earlier on July 22 with the play-off picture falling into place in dramatic style.

West Brom just edged out rivals Brentford and Fulham to the final automatic promotion spot despite all three failing to win on the final day.

Meanwhile, Swansea stunned Nottingham Forest to gatecrash the top six at the last minute and join Welsh foes Cardiff.

The play-offs are now under way with the teams battling it out to become the third and final side to be promoted.

Brentford overcame a first-leg defeat to beat Swansea in their final ever appearance at Griffin Park and reach Wembley.

In the second semi-final, Fulham held off a valiant fight back from Cardiff to book their place.
